Meet Dr. Crippen, author of "ICU Resource Allocation in the New Millennium"

The majority members of the global medical village have learned by trial and error how to indemnify most of their citizens for illness utilizing limited resources and prioritization (saying no to some). The notable exception has been the USA which still adheres to a "customer satisfaction" maxim, creating demand and then supplying it in an open ended fashion with little notice as to cost.
